**Job Summary** We are seeking a dedicated and energetic Chronic Care Partnership Provider, including Advanced Practice Registered Nurses (APRNs), Physician Assistants (PAs), and Medical Doctors (MDs), to join our dynamic healthcare team. In this role, you will collaborate with a multidisciplinary network of healthcare professionals to deliver comprehensive, patient\-centered care for individuals with complex chronic conditions. Your expertise will help improve health outcomes, enhance quality of life, and promote proactive management of long\-term health issues through innovative care models and community engagement. This position offers an exciting opportunity to make a meaningful impact in the lives of diverse patient populations across various care settings. **Responsibilities** * Conduct thorough patient assessments, including vital signs, medical history, and physical examinations, to develop personalized care plans. * Manage chronic conditions such as diabetes, hypertension, heart disease, and respiratory illnesses through medication management, lifestyle counseling, and coordinated care strategies. * Utilize electronic health record (EHR) systems like Epic and eClinicalWorks to document clinical encounters accurately and efficiently. * Collaborate with healthcare professionals across specialties—including gastroenterology, geriatrics, behavioral health, and critical care—to ensure seamless patient care transitions. * Provide telehealth consultations to expand access to specialized chronic care services while maintaining compliance with HIPAA regulations. * Administer injections such as botulinum toxin treatments and dermal fillers; perform procedures like suturing, catheterization, spinal taps, and sterile processing as needed. * Educate patients and caregivers on disease management, medication adherence, nutrition, and lifestyle modifications to promote self\-care and prevent hospital readmissions. * Work with chronic care team: RN, Health Administrator, Community Health workers and supportive care associates. * Lead RPM Services. **Qualifications** * Current licensure as an APRN (with prescriptive authority), PA, or MD in good standing; certification in advanced practice nursing or equivalent required. * Demonstrated experience working with patients managing chronic illnesses within outpatient or inpatient settings; experience in geriatrics or senior care highly preferred. * Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth; strong medical documentation skills essential. * Knowledge of CPT coding, ICD\-10/ICD\-9 coding standards for accurate billing and documentation. * Familiarity with telehealth platforms and virtual patient engagement tools. * Critical care experience from ICU or Level I/II trauma centers is advantageous; experience in emergency medicine or hospital medicine is a plus. * Strong communication skills for client education and interdisciplinary collaboration.
