Oncology Patient Navigator / Support Coach

Prosoma is a growing digital health company improving emotional and behavioral support for people living with cancer. Our programs have reached more than 22,000 patients and are validated through clinical trials and real-world use, with U.S. clinical and health-economic collaborations involving institutions like MemorialCare Todd Cancer Institute and Cedars-Sinai. We are seeking an empathetic, self-motivated Oncology Support Coach to deliver virtual care to patients across the U.S.

In this role, you will deliver structured digital coaching and navigation through our Living Well Plus program, which pairs a mobile app with high-touch human support. You will empower patients using evidence-based Cognitive Behavioral Therapy (CBT) principles and mind-body resilience tools to help them manage distress, build coping skills, and navigate their cancer journey.

Requirements

• Health Coaching \& Clinical Background: Credentialed health coach (NBC-HWC preferred or in progress), social worker (MSW/LCSW), nurse, or health educator with formal training in Motivational Interviewing and behavior change.
• Oncology \& Health Experience: Experience supporting individuals affected by cancer or chronic illness through health coaching, patient navigation, physical therapy support, or clinical social work.
• Navigation Training Requirement: Completion of (or willingness to complete during onboarding) the free, self-paced online GW Cancer Center Oncology Patient Navigator Training: The Fundamentals (\~14 hours) to meet standardized navigation competencies.
• Scope \& Safety Mastery: Deep understanding of coaching scope boundaries, ethical standards, HIPAA, consent, and safety escalation procedures (keeping coaching distinct from formal psychotherapy or medical advice).
• CRM \& Tech Comfort: Experience working out of CRMs, EHRs, or digital health platforms daily to record sessions, log notes, and track billable time.
• Communication \& Emotional Intelligence: Exceptional active listening, empathy, cultural humility, and the ability to support patients through health anxiety, treatment burden, and grief.

Responsibilities

• Patient Coaching \& Engagement: Conduct 1-on-1 virtual coaching sessions aligned with Living Well Plus, using Motivational Interviewing, CBT-informed coping strategies, active listening, and goal-setting to support stress management, treatment resilience, and program engagement.
• Care Navigation \& Empowerment: Help patients identify barriers to care, organize health concerns, prepare questions for oncology visits, access approved resources, and navigate complex health needs.
• Care Plans \& Personalized Goals: Co-create practical, patient-led goals and populate Care Plans in our CRM related to coping, daily routines, app engagement, and social support.
• Monitoring \& Safety Escalation: Monitor standardized engagement signals and distress metrics (e.g., NCCN Distress Thermometer, PHQ-9, GAD-7) and execute protocol-driven safety escalations when needed.
• CRM \& Administrative Systems: Maintain prompt, accurate documentation of patient interactions, call recordings, time tracking, and Care Plans within designated HIPAA-compliant CRM platforms.
• Script \& Program Feedback: Provide ground-level feedback from patient sessions to help refine coaching scripts, workflows, and care pathways as the U.S. program grows.
• Future Peer Mentorship: As the U.S. coaching team expands over time, assist with onboarding, mentoring, and quality reviews for incoming coaches.

Working conditions

• Compensation: $25.00-$30.00 per hour (hourly contract).
