Transform Care.** ----------------------------------------- Are you a board\-certified or board\-eligible Psychiatrist looking for an opportunity to make a meaningful impact through integrated behavioral healthcare? Carbon Health is looking for a **Per Diem Psychiatric Consultant** to join our **Collaborative Care Model (CoCM)** team. This fully remote role offers the opportunity to support patients at scale by partnering with Primary Care Providers (PCPs) and Behavioral Health Care Managers (BHCMs) to provide expert psychiatric consultation and evidence\-based treatment recommendations. Rather than providing traditional one\-on\-one psychiatric care, you will serve as a psychiatric consultant to the care team. Your primary focus will be conducting **systematic, population\-based weekly caseload reviews** of patients enrolled in the Collaborative Care program, helping the team identify patients who are not improving as expected, require diagnostic clarification, or may benefit from adjustments to their treatment plan. This is an opportunity to use your psychiatric expertise to support high\-quality behavioral healthcare across a broad patient population while working collaboratively with an interdisciplinary clinical team. **Compensation \& Schedule** ---------------------------- * **$150–$170 per hour** * **Per Diem** * Up to **8 hours of availability per week** * **Weekdays only — no weekends** * Fully **remote** position Compensation offers are based on factors including education, experience, work history, and geographic location. **Why This Role?** ------------------ ### **Make an Impact at Scale** Instead of focusing on one patient at a time, you'll use your psychiatric expertise to support a broader population of patients receiving behavioral healthcare through primary care. ### **Partner with an Integrated Care Team** Work closely with **Behavioral Health Care Managers and Primary Care Providers** to provide psychiatric consultation, diagnostic guidance, and evidence\-based treatment recommendations. ### **Use Data to Guide Care** Leverage patient registries and validated measurement tools such as **PHQ\-9 and GAD\-7** to monitor treatment progress and help the care team make informed, measurement\-based treatment decisions. ### **Work Flexibly and Remotely** This is a fully remote, per diem opportunity with **up to 8 hours of availability per week**, scheduled during weekdays. **What You'll Do** ------------------ ### **Psychiatric Consultation** * Participate in regular weekly caseload consultation sessions with Behavioral Health Care Managers. * Review patients who are new to the Collaborative Care program or who are not improving as expected. * Provide diagnostic clarification and evidence\-based psychiatric recommendations based on information available in the patient registry and Electronic Health Record (EHR). * Provide concise, actionable recommendations to Primary Care Providers regarding psychiatric treatment. * Recommend appropriate medication selection, dosing, titration, monitoring, and management of potential side effects. * Provide guidance on brief, evidence\-based behavioral strategies that can be incorporated into primary care. * Support the care team in identifying and addressing clinically complex behavioral health presentations. ### **Population\-Based Care \& Caseload Review** * Conduct systematic, population\-based reviews of a designated patient registry. * Use registry data and validated clinical measures, including **PHQ\-9 and GAD\-7**, to monitor patient progress and treatment outcomes. * Prioritize patients who are not improving as expected, present diagnostic or treatment challenges, or require additional clinical guidance. * Identify opportunities for treatment adjustments based on measurement\-based care principles. * Collaborate with the Behavioral Health Care Manager to establish priorities and follow\-up actions for patients reviewed during weekly caseload consultations. * Document recommendations and communicate relevant treatment guidance to the care team. ### **Care Coordination \& Specialty Referral** * Identify patients whose clinical complexity, risk, or treatment needs exceed the scope of primary care\-based behavioral health treatment. * Provide guidance regarding appropriate specialty psychiatric referrals and community\-based mental health resources. * Support the care team in determining when a patient may benefit from a higher level or more specialized level of psychiatric care. * Collaborate with PCPs and BHCMs to promote continuity of care and appropriate transitions to specialty behavioral health services. ### **Clinical Collaboration \& Education** * Partner effectively with PCPs and BHCMs as part of an interdisciplinary Collaborative Care team. * Serve as a clinical resource for non\-psychiatric providers managing behavioral health conditions in the primary care setting. * Provide education and clinical guidance related to psychiatric diagnoses, medications, differential diagnosis, and evidence\-based treatment approaches. * Communicate recommendations clearly, efficiently, and respectfully to support collaborative decision\-making. * Participate in process improvement initiatives related to the Collaborative Care program as needed. **Why Carbon Health?** ---------------------- At Carbon Health, we're transforming healthcare by combining exceptional clinical care, innovative technology, and a collaborative culture. You are comfortable using your psychiatric expertise to provide focused, actionable recommendations and are excited by the opportunity to improve care for a population of patients rather than providing traditional one\-on\-one psychiatric treatment. ### **Qualifications** * Doctor of Medicine (**MD**) or Doctor of Osteopathic Medicine (**DO**) from an accredited institution. * **Board\-certified or board\-eligible in General Psychiatry.** * Active and unrestricted medical licenses in **Massachusetts and New Jersey**. * Valid and unrestricted **DEA registration**. * Strong clinical knowledge of psychiatric diagnosis and evidence\-based treatment. * Excellent communication and interpersonal skills. * Demonstrated ability to collaborate effectively with Primary Care Providers, Behavioral Health Care Managers, and other healthcare professionals. * Strong clinical judgment and ability to provide concise, actionable recommendations based on available patient information. * Comfort working with Electronic Health Records (EHRs) and patient registries. * Ability to work efficiently within structured weekly consultation time blocks. ### **Preferred Qualifications** * Experience practicing within the **Collaborative Care Model (CoCM)**, including the **AIMS Center model**, or another integrated behavioral health setting. * Experience providing psychiatric consultation to primary care clinicians. * Experience with population\-based behavioral health care and registry\-based patient tracking. * Familiarity with measurement\-based treatment using tools such as **PHQ\-9 and GAD\-7**. * Experience supporting medication management within a primary care or integrated care environment. * Experience identifying patients who may require specialty psychiatric services or a higher level of care.
